Say you have high blood pressure. There's a new blockbuster drug on the market, and your doctor lets you know about a new clinical trial you can join that is testing the new treatment against an old tried-and-true one. What's not to like? You're going to be taking, under the care of experts, one of two U.S. Food and Drug Administration–approved medications.
What you might not know—even after you sign up for the trial and have inked the informed-consent form—is that scattered reports are starting to suggest that the new medication might occasionally cause severe side effects. And the real reason the trial is being conducted with these previously released drugs is to test whether the new medication really is a lot riskier to everyone or just to a subset of patients./.../

Pacientes e médicos contarão, a partir desta sexta-feira (31), com regras que estabelecerão os critérios sobre o uso de tratamentos considerados invasivos ou dolorosos em casos clínicos nos quais não exista qualquer possibilidade de recuperação. Sob o nome formal de diretiva antecipada de vontade, mas já conhecido como testamento vital, trata-se do registro do desejo expresso do paciente em documento, o que permitirá que a equipe que o atende tenha o suporte legal e ético para cumprir essa orientação.
 
A regra consta da Resolução 1.995, aprovada pelo plenário do Conselho Federal de Medicina (CFM), que será publicada no Diário Oficial da União no dia 31 de agosto. Assim, o paciente que optar pelo registro de sua diretiva antecipada de vontade poderá definir, com a ajuda de seu médico, os procedimentos considerados pertinentes e aqueles aos quais não quer ser submetido em caso de terminalidade da vida, por doença crônico-degenerativa./.../

In one sense, science educators have it easy. The things they describe are so intrinsically odd and interesting — invisible fields, molecular machines, principles explaining the unity of life and origins of the cosmos — that much of the pedagogical attention-getting is built right in.  Where they have it tough, though, is in having to combat an especially resilient form of higher ed’s nemesis: the aptly named (if irredeemably clichéd) ‘preconceived idea.’ Worse than simple ignorance, naïve ideas about science lead people to make bad decisions with confidence. And in a world where many high-stakes issues fundamentally boil down to science, this is clearly a problem. /.../

Transcrevo a nota de falecimento do FOGUINHO, escrita pelo colega, também grande sanitarista, Jair Ferreira, que teve com ele maior contato.
Tive também diversos encontros na vida com o Cláudio: no Projeto de Análise Institucional da SSMA, depois quando apresentei meu projeto de Programa de Prevenção da Febre Reumática, posteriormente no IPB na identificação dos Estreptococos e num projeto frustrado para determinação da prevalência de infecção estreptocócicas em escolares de Porto Alegre, ainda, enquanto na OPAS em diversas oportunidades e como cliente até que teve sua valvulopatia reumática operada no IC, o que lhe rendeu mais alguns anos de vida.
A última vez que o encontrei foi no lançamento do livro do Eduardo Costa, na Assembléia Legislativa no ano passado. Estava com aparência muito boa e, como sempre, alegre.
Mais um que nos deixa da presença física, mas também nos deixa um legado inestimável.

Colegas do DMS

Faleceu hoje o Dr. Cláudio Marcos da Silveira, sanitarista gaúcho que se destacou em várias áreas. Ele foi Diretor do LACEN (na época IPB) e Secretário Municipal da Saúde nos dois primeiros anos do Prefeito Collares (ele era filiado ao PDT). Seus maiores feitos, entretanto, deram-se na área do controle e erradicação de doenças infecciosas. Ele participou , com outros sanitaristas gaúchos notáveis, da criação do Sistema de Vigilância Epidemiológica das doenças transmissíveis que, por muito tempo, foi modelo para o Brasil.  Foi este grupo comandou as ações que levaram à erradicação da varíola no RS em 1970 e forneceu a base para a erradicação da pólio na década de 1980.  Não bastassem todas essas atividades de coordenação, ele também foi um homem da “linha de frente”. Como consultor da OMS, participou da erradicação da varíola em países como Bangladesh e Somália, vacinando populações nômades ou que viviam em locais de difícil acesso, permanecendo meses em acampamentos no interior desses países. Mais tarde, fez trabalhos semelhantes no combate à Pólio. Foi condecorado pela OMS com a “Ordem da Agulha Bifurcada”, por sua contribuição na erradicação mundial da varíola. No Brasil, contraiu malária, combatendo essa doença no interior da Amazônia. Era uma pessoa simples que nunca fez autopromoção. Grande colega de profissão e de mesa, tomando um “chops”. Faço aqui um tributo à sua trajetória e seu eterno bom humor.  Abração.

Chronic cough affects 11 percent to 16 percent of the population, the researchers report. The exact cause is unknown, but it might relate to a malfunction in a part of the brain that causes coughing. Gabapentin works by suppressing that "cough center," they said.
TUESDAY, Aug. 28 (HealthDay News) -- A drug commonly used to control epileptic seizures and pain, gabapentin, also appears to ease hard-to-treat chronic coughs, a new study says.
"It is effective and well-tolerated, and may provide significant relief and respite from such a physically and psychologically disabling condition," said lead researcher Nicole Ryan, a clinical research scientist at the University of Newcastle in Australia. "Gabapentin is a real treatment option for people with refractory chronic cough, especially for those with features of central sensitization."
People with this kind of cough feel the need to cough when there is no cough stimulus present. It's thought that neurons in the central nervous system fire off unnecessarily.

The preferred initial treatment for patients with stable coronary artery disease is the best available medical therapy. We hypothesized that in patients with functionally significant stenoses, as determined by measurement of fractional flow reserve (FFR), percutaneous coronary intervention (PCI) plus the best available medical therapy would be superior to the best available medical therapy alone.

CONCLUSIONS

In patients with stable coronary artery disease and functionally significant stenoses, FFR-guided PCI plus the best available medical therapy, as compared with the best available medical therapy alone, decreased the need for urgent revascularization.
